Shared Decision-Making in the Management of Urolithiasis

Urolithiasis affects approximately 10% of the global population, with recurrence rates exceeding 50% within five years despite effective medical and surgical interventions. While technological advances have improved stone treatment, long-term success depends heavily on patient adherence to preventive strategies, including dietary modifications, fluid intake and pharmacological regimens. Adherence, however, remains suboptimal and little is known about how patient preferences, values and beliefs influence treatment choices and health behaviours across the care continuum. This Article Collection aims to explore the intersection of urolithiasis management with patient-centred outcomes, including treatment acceptance, lifestyle adherence and satisfaction with shared decision-making. We invite research that moves beyond clinical efficacy to understand what matters most to patients living with recurrent stone disease.
Poor adherence to preventive measures in urolithiasis leads to unnecessary morbidity, repeated interventions, increased healthcare costs and diminished quality of life. Traditional clinical guidelines often overlook the psychosocial, economic and cultural factors that shape patient behaviour. Understanding patient preferences is critical, for example, whether patients favour watchful waiting overactive intervention or how side effects influence adherence to potassium citrate or thiazide diuretics. Furthermore, the rise of telemedicine, mobile health apps and personalized care models offers new opportunities to support adherence, but their effectiveness depends on alignment with patient needs. By centreing patient perspectives, this Collection will generate evidence to inform guideline development, improve counselling practices and reduce the burden of recurrent stone disease. It addresses a key gap in urological care: moving from a purely biomedical to a biopsychosocial model of chronic stone management.
This Collection aligns with the scope of Patient Preference and Adherence by focusing on patient-reported outcomes, treatment adherence and decision-making in urolithiasis. The subtopics listed below are intended as illustrative examples rather than strict categories; other submissions on related aspects of the topic are equally welcome. Suitable subtopics include (but are not limited to): patient preferences for surgical vs. medical vs. observational management; barriers and facilitators to dietary/fluid adherence; role of health literacy, risk perception and treatment beliefs; shared decision-making tools and decision aids; digital health interventions to support self-management; socioeconomic and cultural influences on adherence; and quality of life in recurrent stone formers. Accepted article types include original research, systematic reviews, qualitative studies, study protocols and short reports. We particularly welcome cross-sectional surveys, mixed-methods studies and intervention trials that incorporate validated adherence or preference measures.
