Clinical pharmacy has undergone significant transformation in recent years from a product-focused discipline to a patient-centered, driven by technological advancements and a growing emphasis on optimizing medication therapy and improving health outcomes. The integration of artificial intelligence (AI) and machine learning algorithms has also the growing emphasis on personalized therapy present key challenges in diverse healthcare systems. New technologies, precision medicine, and data-driven clinical decision support systems are changing how pharmacists contribute within multidisciplinary teams. The expansion of pharmacists’ roles in chronic disease management, antimicrobial stewardship, and personalized care reflects a commitment to integrating evidence-based practice into clinical settings. Advancements in pharmacogenomics, tele pharmacy, and AI-enabled tools further enhance pharmacists’ capacity to deliver safe, effective, and efficient care.

This article collection, which welcomes research presented at the 24th Asian Conference on Clinical Pharmacy (The 24th ACCP) July 24 - 27 2025, seeks to capture these developments, highlighting research, policy analyses, and innovative practices that shape the future of clinical pharmacy and its impact on healthcare systems worldwide.

The advancement of clinical pharmacy is critical in addressing global healthcare challenges, including medication safety, rising healthcare costs, and increasing demands for patient-centered care.
